University project about NOVA - Can you Help?

I am currently studying multimedia at university currently, and I am wondering if I could get any of you people on here to give me a hand.
One of my modules currently is Interaction and Usability, and for the project I have chosen to create a prototype replacement for the Nova system.
To help me assess the needs of users, I have compiled a short survey. It should take no more than 5 minutes to complete, and if you could, I would be very grateful.

I have filled in the survey, Kev.
Would it be better if the respondent was given a chance to enter their email address, so that you can follow up any suggestions made.

I may have it wrong, but I got the impression that the final page where it asks for your details is for the benefit of freeonlinesurveys.com rather than for you.

I have also completed the survey, just wanted to say that it was a good survey, with some very straight to the point questions, I wish more were like it.
