A few questions about booking with kids/infants

About a month ago I booked a rather complicated combination of tickets for next years trip to the in-laws in MCO

Basically it went something like this:

1 L class ticket for myself to ensure eligibility for...
1 Free companion ticket for the missus
1 Q class ticket for our 2 year old (her first seated ticket) - and she gets miles on the family account
1 infant ticket for our unborn on miles - no name yet so have to call back on that one

Now obviously I had to phone VS to do this as I needed to know the fare for a qualifying L class ticket and book the companion ticket aswell as there being no facility online to book infant tickets on miles. This sort of irked me a bit as I could not pick up the online booking bonus. But it started me thinking of a few things that I wonder if anyone has any comments on.

1. Could I have booked the Q and L tickets online (to get the mileage bonus) and then called to add the companion?
2. Failing that could I have booked the L and companion by phone and then the Q for the child online and linked it to the others - or would it reject a single kid booking?
3. Why can you not book infant miles tickets online when you can book infant money tickets online?

And lastly a point to ponder. Having made the decision to buy L class for the companion ticket rather than use miles I agonised over whether to use miles for the youngster but the fact that you have to pay a full adult mileage price means that it devalues your miles. Why do not VS offer a child reduction in miles? Do any airlines? (Incidentally Nectar does have reduced rates for kids so it makes it worthwhile going this route but the dates where no good for the offer)

Any thoughts?
