Continental is the latest to make a move designed to reward its customers who pay the highest fares. The carriers has come up with a "No Middle Seat" guarantee for full-fare paying travelers. The deal applies to OnePass members traveling on non-discounted domestic fare codes starting with A8/D8/Y8/YA3 and ticketed in A/D/Y class of J/D/Z/C/A/Y service. For international service, the deal applies to non-discounted fare codes starting with J/D/Z/C/A/Y and ticketed in J/D/Z/C/A/Y class. Travelers who meet the booking requirements but still get a middle seat will be rewarded a maximum of 2,500 bonus miles one-way.

I used to be American Platinum (then VS started flying where I needed them ) and we used to be guaranteed a blocked out middle seat if flying coach. Don't know if it still applies as I've dropped to Au with them and pretty much never fly them anymore.
